About Senior Hospice
A serious, advancing life-limiting illness can be a difficult condition to confront--let alone to talk about with your loved ones and friends. Yet, for many of us, the day will come when we need to confront limitations resulting from serious illnesses and consider how we would like to be treated if our condition became terminal. How might we like our life’s final chapters to unfold?
The Senior Hospice team advocates for the rights of terminally ill patients and their families. We believe that patients should be apprised of their clinical prognosis and be involved in the decision-making process that determines how their end-of-life care is provided.
The Healthcare Team
NURSES are experienced in assessing and managing pain; they provide hands-on care.
SOCIAL WORKERS are friendly and skilled listeners who can offer emotional support and help with financial planning and Bereavement Services.
PHYSICIANS collaborate with the patient’s primary physician and are available for pain and symptom management consultations, as well as to make house calls as needed.
CERTIFIED HOSPICE AIDES help with personal care, such as bathing and mouth care; they also can help with light cleaning, light laundry and occasional shopping.
CHAPLAINS offer spiritual support.
VOLUNTEERS are important elements of the team who are available when they are needed most.
CONTRACTED DIETICIANS help patients achieve the best nutritional standards given the current state of his or her health. Dieticians have much experience in dealing with the dietary challenges that often affect the terminally ill. In conjunction with the physician, dieticians will advise on meal plans, food preparation and even on how to serve the patient properly.
All patients entering hospice care will be evaluated at the time of admission into program. If there is a need for nutritional assistance, a dietician will be provided to assist and educate patients and their families.
